What Makes a Successful Interprofessional Team?

Views from Health Service Providers in Northwestern Ontario

This study was conducted to determine what interprofessional (IP) models are being used across the North West LHIN.  This scan provides evidence related to the potential benefits of IP education (IPE) and IP care (IPC). More importantly, this scan identifies the themes and determinants associated with successful collaborative patient centred practice. Ten focus groups and four key informant interviews were held between January 1 and March 1, 2009. Individuals from both the health care and health education sectors were asked to participate in focus groups or as key informants. Efforts were made to attain input from a broad range of health professions, health care organizations and geographic region within the North West LHIN.
