Building a High Performance Diabetes Health Team

Diabetes is a chronic, often debilitating and sometimes fatal disease that impacts the health status of people of Northwestern Ontario. Diabetes related complications can be life threatening or significantly impact a person’s quality of life. Through comprehensive diabetes care and management it is possible to prevent or delay the onset of these complications. The Regional Diabetes Plan supports advances in diabetes care to create a system of care that is preventive, integrated and person-centred and ensures smooth transitions throughout the patient journey. A health and wellness model with a focus on facilitating self-management will utilize technology and foster innovation to optimize equitable access to diabetes care through a multidisciplinary team approach. The goal of the Regional Plan is to mobilize, strengthen and reorient the health care system to improve access to safe, comprehensive and high quality diabetes care for all people of the North West Local Health Integration Network (LHIN) region. By achieving this goal, health outcomes are improved, enhancing quality of life for people living with diabetes, those at risk of developing diabetes and their families. 

The Regional Diabetes Plan was informed by qualitative and quantitative information including stakeholder engagement with decision makers and front-line providers. Relevant evidence-informed models of care were identified through a literature review and scan of key documents from other jurisdictions. 

Recommendations of the Regional Diabetes Plan are aligned with Ontario’s Action Plan for Health Care, Ontario’s Diabetes Strategy, and the North West LHIN’s Integrated Health Services Plan 2016-2019 and Health Services Blueprint. The service delivery model proposed for diabetes care aligns with the North West LHIN Health Services Blueprint by outlining elements for implementation at the Local Health Hub (LHH), Integrated District Network (IDN), and Regional levels.
